By submitting photos to the Eveleth Public Library you are agreeing to the following:
The Eveleth Public Library has my permission to use my or my child’s/children’s photograph publicly to promote the library. I understand that the images may be used in various forms, including but not limited to, print publications, online publications, presentations, websites, and social media. I understand that my child’s/children’s picture, name, and/or age only may appear in such publicity material. No home address or telephone number will appear in such work.
I hereby irrevocably authorize the Eveleth Public Library to edit, alter, copy, exhibit, or distribute my photograph for the purposes of publicizing the Library’s programs, or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product.
I also understand that since my participation is voluntary, no royalty, fee, or other compensation shall become payable to me by reason of such use and I hereby waive any right to such royalties.
I hereby hold harmless and release and forever discharge the Eveleth Public Library from all claims, demands, and causes of action that I, my heirs, or any other persons acting on my behalf or behalf of my estate may have. All photo/information release forms will be kept on file at the library for one calendar year.
